The regulation of lockout and tag out management (recommended by safe -protection Specialist )

1. Purpose
To prevent the power system operation accidentally during the maintenance, adjusting or upgrading. And it will caused accident that the operator hurt by release hazard energy (such as electricity, compress air and hydraulic etc)

2. Scope
The process of tag out and lock out as below.
a) The assignment connecting with power system, such as electricity, pneumatic, hydraulic devices.
b) Non – repetitive, non – routine installation and commissioning.
c) To connect the power of device by plug.
d) The Switch device in the repairing site which can not see the power line.
e)The place where will release the hazard energy (including the electricity, chemical, pneumatic, mechanical, heat, hydraulic, spring-return and falling weight).
Except power sockets within the scope of operator control.

3. Definition
a. Licensed operation/personnel: the person who can lock out, remove the lock and restart energy or equipment in the locking procedure.
b. Related personnel: the person who are engaged in lockout in the maintenance of equipment.
c. Other personnel: the person who are working around the lockout control device but have no relation to this control device.

4. Duty
a. The duty officer in each departments have responsible to implement the provisions and appointed the person to lockout /tag out.
b. Engineer and equipment maintenance personnel in each department are responsible for make list of devices which need to lockout and tag out.
c. General office to develop system of lockout and tag out.

5. Management requirements or specifications
5.1 requirements
5.11 The concessionaire shall disconnect the switch of the power supply line and lock out. Before repair of process equipment or power line. It should be tag out on the maintained equipment to indicates it is in repairing. For example, The power plug can be without lock when it is one source of usage within the control scope, but must be tag out. And power supply is necessary for maintenance or equipment debugging, it can tag out without lock and there is a guardian on the spot to fill out .
5.1.2 The maintainence, part should be disconnected power supply and disassemble from the maintenance equipment. And that includes disassembly a transmission device for conveying power, such as a belt, chain, coupling, etc.
5.1.3 To purchase the device which can be lockout when it need to be replaced.
5.2 Locks: Maintenance locks include padlocks and perforated locking plates, the lock is kept by a licensed worker. Only one key available, it can use multiple holes lock plate when the maintenance involving many operators.
5.3 Lockout and tag out at the meanwhile and warning the other people do not remove the lock.
5.4 The lock and tag only can be remove by authorized person.
5.5 The authorized person can not operate the lockout and tag out device in case of shift change or replacement.
5.6 It indicates that the device is operating by multiple workers when there are many locks on the plate.
5.7 Company employees are strictly prohibited to remove locks without permission. When there are outside suppliers working on the company site and lockout or tag out.
5.8 Operating instruction.
5.8.1 Preparation before shutting down.
a. Notify personnel to check.
b. Make clear of the type and quantity, risk and the control method of energy.
5.8.2 Device shutdown/ isolation of power.
a. Shut down the device according to the operating instructions.
b. Ensure isolation of all energy that can enter the facility.
5.8.3 Lockout/tag out applications.
a. How to use the tag /lock supplied by the company?
b. Must be tag out or adopt other secure measures if it can not lockout, and wear protective equipment to eliminate hidden dangers.
5.8.4 Control of existing energy sources
a. Check all working parts to make sure they stop working.
b. Support relevant equipment/components well to prevent gravity from triggering energy.
c. Release of superheated or super cooled energy.
d. Clean residues in process lines.
e. Close all valves and isolate with blind plate when no valve is available.
5.8.5 Confirm isolation device status.
a. Confirm isolation device status.
b. Make sure the energy control switch can no longer be moved to the “on” position.
c. Press the device switch and the test cannot be started again.
d. Check other isolation devices.
e. Place all switches in the “off” position.
f. Electricity testing.
5.8.6 Repair work.
A. Avoid restarting the power switch before work.
B. Do not bypass existing lockout/tag out device when installing new piping and circuitry.
5.8.7 Remove the lock and tag.
